If you are planning to register your child at Millstream Elementary, you may want to download the necessary forms and fill them in at home. Please note that you will need to have Adobe Acrobat Reader in order to view and print these documents. They can then be brought to school on the day you register your child. Below are the forms you will need...
You will need to bring with you:
-> The student’s birth certificate for proof of age
(Photo copy not acceptable)
-> The student’s BC Medical Care Card
(NOTE: Care Card cannot be accepted as proof of age)
-> Proof of residence
( document verifying the address where the student resides...e.g. BC Hydro bill)
PLEASE NOTE:
Your child may not be placed in a classroom on the day you register.
Staff often need time to make arrangements
for the new student.
The staff will work to make arrangements as quickly as possible.
